Congratulations and thank you for your\nsupport of the Pittsburgh Regional Science and Engineering Fair and the\nstudents participating in the fair!&nbsp; <\/p>\n\n\n\n<p><strong>Designated Supervisor Role<\/strong><\/p>\n\n\n\n<p>The Designated Supervisor is the responsible adult who will\nbe providing direct supervision to the student while they are completing their\nexperimentation.<\/p>\n\n\n\n<p>If the experiment is\nbeing completed at home, the designated supervisor may be a parent or may be\nanother responsible adult in the household who will be present while the\nstudent completes their experiment. &nbsp;This\nDesignated Supervisor will need to sign <em>Form 3: Risk Assessment Form<\/em>.<\/p>\n\n\n\n<p>If the experiment is\nbeing conducted in a regulated research institution, but not under the direct\nsupervision of a Qualified Scientist, the Designated Supervisor may be a\ngraduate student, post-doctoral fellow or other member of the Qualified\nScientist\u2019s team who is assigned to supervise the student. This Designated\nSupervisor will need to sign <em>Form 3: Risk Assessment<\/em> <em>Form<\/em> and,\ndepending on the location of the experimentation and topic of the research, <em>Form\n2: Qualified Scientist Form<\/em>, <em>Form 5A\/B: Vertebrate Animal Form, Form 6A:\nPotentially Hazardous Biological Agents Form, Form 6B: Human and Vertebrate\nAnimal Tissue Form <\/em>and\/or<em> Form 1C: Regulated Research\nInstitution\/Industrial Setting Form<\/em>.<\/p>\n\n\n\n<p>As stated by the\nSociety for Science and the Public\u2019s International Rules: Guidelines for\nScience and Engineering Fairs 2020\u20132021 (<a href=\"https:\/\/www.societyforscience.org\/isef\/international-rules\/\">https:\/\/www.societyforscience.org\/isef\/international-rules\/<\/a>):<\/p>\n\n\n\n<p>\u201cThe Designated\nSupervisor (DS) Qualifications: <\/p>\n\n\n\n<p>\u2022 Does not need an advanced degree <\/p>\n\n\n\n<p>\u2022 Must be familiar with the student\u2019s project\nand agree to any training necessary<\/p>\n\n\n\n<p>\u2022 May also serve as the Adult Sponsor for the\nproject <\/p>\n\n\n\n<p>\u2022 If the project involves the use of\nVertebrate Animals (where behavior\/habitat is influenced by humans), must be\nknowledgeable about the humane care and handling of the animals <\/p>\n\n\n\n<p>Responsibilities: <\/p>\n\n\n\n<p>\u2022 Providing direct supervision of the student\nexperimentation<\/p>\n\n\n\n<p>\u2022 Completing the required documentation\u2009\u2014\u2009the\nDesignated Supervisor box on the Qualified Scientist Form (2) when applicable<\/p>\n\n\n\n<p>\u2022 Reviewing and completing the Risk\nAssessment Form (3) when needed\u201d<\/p>\n\n\n\n<p><strong>Logging In<\/strong><\/p>\n\n\n\n<p>In order to sign the required form(s) for the student(s), you will need to log in to your account at <a href=\"https:\/\/www.STEMisphere.org\/PRSEF\">https:\/\/www.STEMisphere.org\/PRSEF<\/a>.&nbsp; <\/p>\n\n\n\n<p>An account was\ncreated for you when the student or their Adult Sponsor added your name to the\nstudent\u2019s profile.&nbsp; You should have\nreceived an email from <a>CSC STEMisphereInfo at <\/a><a href=\"mailto:STEMisphereInfo@CarnegieScienceCenter.Org\">STEMisphereInfo@CarnegieScienceCenter.Org<\/a> with\ninformation about how to log in.&nbsp; Please\ncheck your junk mail folder if you do not see this email.&nbsp; <\/p>\n\n\n\n<p>Use the link in the\nemail to set your password and log in. If you have been assigned to more than\none student\u2019s project, you will see them each listed on your screen. In this case, the Designated Supervisor may be a graduate\nstudent, post-doctoral fellow or other member of the Qualified Scientist\u2019s team\nwho is qualified to supervise the student and who has been chosen by the\nQualified Scientist to do so.<\/p>\n\n\n\n<p>A\nRegulated Research Institution is defined as an industrial laboratory,\nprofessional research\/teaching institution which is regularly inspected by the\nUSDA, college\/university laboratory, hospital or medical center, federal\nlaboratories like those of the NIH, CDC and veteran\u2019s affairs, pharmaceutical\nand biotechnology company laboratory, or research institution that utilizes\nresearch animals which is not covered by the Animal Welfare Act but has an\noperational Institutional Animal Care and Use Committee (IACUC) and is in\ncompliance with U.S. federal laws.<\/p>\n\n\n\n<p>A\nQualified Scientist is defined as a person who has earned a\ndoctoral\/professional degree in a scientific discipline related to student\u2019s\narea of research and\/or has extensive experience and expertise in the student\u2019s\narea of research and is thoroughly familiar with the regulations that govern\nthe student\u2019s area of research.<\/p>\n\n\n\n<p><em>Form 5A\/B:\nVertebrate Animal Form<\/em> must be signed by a Designated Supervisor when a\nstudent\u2019s project involves the use of vertebrate animals. Form 5A is used when\nthe research is conducted outside of a Regulated Research Institution.&nbsp; The Designated Supervisor is the individual\nwho will directly supervise the student during their experiment and will take\nprimary responsibility for the care of the animals during the study.&nbsp; <\/p>\n\n\n\n<p>The Designated\nSupervisor may be a parent if the project is being conducted in the student\u2019s\nhome, a teacher if the experiment is being conducted at school, or another\nresponsible adult if the experiment is being conducted in the field.&nbsp; <\/p>\n\n\n\n<p>If the vertebrate\nanimal study is being conducted in a Regulated Research Institution, Form 5B\nVertebrate Animal Form may be signed by the Qualified Scientist. <\/p>\n\n\n\n<p>A\nRegulated Research Institution is defined as an industrial laboratory,\nprofessional research\/teaching institution which is regularly inspected by the\nUSDA, college\/university laboratory, hospital or medical center, federal\nlaboratories like those of the NIH, CDC and veteran\u2019s affairs, pharmaceutical\nand biotechnology company laboratory, or research institution that utilizes\nresearch animals which is not covered by the Animal Welfare Act but has an\noperational Institutional Animal Care and Use Committee (IACUC) and is in\ncompliance with U.S. federal laws.<\/p>\n\n\n\n<p>If the experiment is\nbeing conducted in a regulated research institution, but not under the direct\nsupervision of a Qualified Scientist, a Designated Supervisor may sign this\nform. The Qualified\nScientist may appoint another person as a Designated Supervisor. The Designated\nSupervisor is the person who is providing direct on-site supervision to the\nstudent while they complete their project. Or 2) the student is working with\npotentially hazardous biological agents outside of a Regulated Research\nInstitution under the supervision of their Adult Sponsor or other adult who is\nfamiliar with the project and is able to provide necessary training and safety\nmeasures. The adult providing the direct supervision to the student may fill\nthe role of Designated Supervisor.<\/p>\n\n\n\n<p>A\nRegulated Research Institution is defined as an industrial laboratory,\nprofessional research\/teaching institution which is regularly inspected by the\nUSDA, college\/university laboratory, hospital or medical center, federal\nlaboratories like those of the NIH, CDC and veteran\u2019s affairs, pharmaceutical\nand biotechnology company laboratory, or research institution that utilizes\nresearch animals which is not covered by the Animal Welfare Act but has an\noperational Institutional Animal Care and Use Committee (IACUC) and is in\ncompliance with U.S. federal laws.<\/p>\n\n\n\n<p>A\nQualified Scientist is defined as a person who has earned a doctoral\/professional\ndegree in a scientific discipline related to student\u2019s area of research and\/or\nhas extensive experience and expertise in the student\u2019s area of research and is\nthoroughly familiar with the regulations that govern the student\u2019s area of research.<\/p>\n\n\n\n<p><em>Form 6B: Human\nand Vertebrate Animal Tissue Form <\/em>must be signed by a Designated Supervisor\nwhen 1) the student is working with human and vertebrate tissue in a Regulated\nResearch Institution and the Qualified Scientist associated with the project is\nnot directly supervising the student. The Qualified Scientist may appoint\nanother person as a Designated Supervisor. The Designated Supervisor is the\nperson who is providing direct on-site supervision to the student while they\ncomplete their project. Or 2) the student is working with human and vertebrate\ntissue outside of a Regulated Research Institution under the supervision of\ntheir Adult Sponsor or other adult who is familiar with the project and is able\nto provide necessary training and safety measures. In this case, the Designated Supervisor\nmay be a graduate student, post-doctoral fellow or other member of the\nQualified Scientist\u2019s team who is qualified to supervise the student and who\nhas been chosen by the Qualified Scientist to do so.<\/p>\n\n\n\n<p>A\nRegulated Research Institution is defined as an industrial laboratory,\nprofessional research\/teaching institution which is regularly inspected by the\nUSDA, college\/university laboratory, hospital or medical center, federal\nlaboratories like those of the NIH, CDC and veteran\u2019s affairs, pharmaceutical\nand biotechnology company laboratory, or research institution that utilizes\nresearch animals which is not covered by the Animal Welfare Act but has an\noperational Institutional Animal Care and Use Committee (IACUC) and is in\ncompliance with U.S. federal laws.<\/p>\n\n\n\n<p>A\nQualified Scientist is defined as a person who has earned a\ndoctoral\/professional degree in a scientific discipline related to student\u2019s\narea of research and\/or has extensive experience and expertise in the student\u2019s\narea of research and is thoroughly familiar with the regulations that govern\nthe student\u2019s area of research.<\/p>\n\n\n\n<p>For a detailed\ndescription of each form, its purpose, when it is required, who may complete it\nand who may sign it, see the ISEF Form Guide at <a href=\"https:\/\/www.PRSEF.STEMisphere.org\">https:\/\/www.PRSEF.STEMisphere.org<\/a>.<\/p>\n\n\n\n<p><strong>Form Status<\/strong><\/p>\n\n\n\n<p>All forms will remain in Required (red background) or Pending\nCompletion status (yellow background) until the required fields have been\ncompleted and the appropriate signatures have been added successfully.&nbsp; Once the required information and signatures have\nbeen added, the form will be promoted to Ready for Review status (purple). <\/p>\n\n\n\n<p><strong>Filling multiple roles<\/strong><\/p>\n\n\n\n<p>If you are filling more than one role for one student (parent and designated supervisor for example) or if you are filling different roles for different students (designated supervisor for one and qualified scientist for another), you will need to switch between profiles to access all of the applicable forms. To move between profiles, look to the horizontal menu bar just below the black and white speckled area on your screen. Click on &#8220;Registrations \/ Forms&#8221; and then on &#8220;Manage <em>Role Name<\/em> Forms&#8221; to switch to a new role.<\/p>\n\n\n\n<p><strong>Reviewing the project<\/strong><\/p>\n\n\n\n<p>Before you sign any form which has been assigned to you,\nreview the student\u2019s research plan and the responses to the questions on the\nform with them. The research plan should include a rationale (background\nresearch), a research question, a hypothesis, a procedure, a risk analysis, a\nproposed method of data analysis and a bibliography with at least five (5)\nsources.&nbsp; <\/p>\n\n\n\n<p>Once you are familiar with the project, review the PRSEF\nrulebook and the ISEF rulebook. Both can be found at <a href=\"http:\/\/carnegiesciencecenter.org\/educators\/stemcenter-science-fair-teachers-and-students\/\">http:\/\/carnegiesciencecenter.org\/educators\/stemcenter-science-fair-teachers-and-students\/<\/a>.\n<\/p>\n\n\n\n<p><strong>Completing Form 3<\/strong><\/p>\n\n\n\n<p>This form&nbsp;is required for ALL projects regardless of\nthe subject and division.&nbsp; It is designed\nto uncover any risks to the safety of the student or others involved in the\nproject and to ensure that those risks are addressed in a way which ensures the\nsafety of all parties. This form is required for all projects regardless of the\nlevel of risk involved.&nbsp; <\/p>\n\n\n\n<p>The student researcher should complete this form.&nbsp; Detailed instructions for completing each\nquestion on this form can be found on STEMisphere.org\/PRSEF. The Designated\nSupervisor may provide guidance and assistance.&nbsp;\nEncourage the student to remember that all activities, and all\nexperiments, involve some risk.&nbsp; Even simple everyday activities involve\nsome risk.&nbsp; For example, taking a shower in the morning before going to\nschool involves the potential risk of burns if the water is too hot or slipping\non a wet surface.&nbsp; <\/p>\n\n\n\n<p>If the student has responded \u201cN\/A\u201d or similar to all of the\nquestions, please do not sign the form. The Scientific Review Committee will\nnot approve the project if the form is completed in that manner.&nbsp;&nbsp; Instead, work with the student to approach\nthe question of risk from a position of curiosity.&nbsp; Some risk can be found\nin just about any project.<\/p>\n\n\n\n<p>This form will remain in Pending Completion status until all\nrequired signatures are provided.<\/p>\n\n\n\n<p><strong>Completing and signing other forms<\/strong><\/p>\n\n\n\n<p><em>Provide information<\/em>: Open each form which is assigned\nto you by clicking on the red or yellow box with the number of the form. See\nthe form descriptions and instructions for each question for help completing\neach form. <\/p>\n\n\n\n<p><em>Save and Sign Form<\/em>: Once you have completed each\nform, click on the Sign Form button. On Form 6A and 6B, you may need to click\non Save Form, then re-open the form and click on Sign Form.<\/p>\n\n\n\n<p><em>Confirm responses<\/em>: Wait for the system to load the\nform in the gray box before continuing. Review the form which has loaded in the\ngray box and confirm that your responses were recorded correctly.&nbsp; Click in the box next to the word \u201cYes\u201d to\nconfirm your responses. <\/p>\n\n\n\n<p><em>Add signature<\/em>: Click in the space below the word\nSignature and above the signature line. Use your mouse or touch pad to add your\nelectronic signature in the box.&nbsp; If your\nsignature is not legible, click on the Clear button to try again. If you are\nhappy with your signature, click on the Accept button. You must click on the\nSign button to confirm your signature.<\/p>\n\n\n\n<p><em>Date of Approval<\/em><strong>:<\/strong> Enter the date when the idea\nfor the project was first reviewed with the student researcher and\nexperimentation was approved by the Qualified Scientist or Designated\nSupervisor. This does not need to be today\u2019s date. The Qualified\nScientist or Designated Supervisor must review and approve the project before\nthe student begins experimentation. <\/p>\n\n\n\n<p>Experiments which were completed over the summer or for a\nschool science fair will have experimentation dates which are before the date\nthis form was completed. Use this field to enter the date the project was\nreviewed and approved by the Qualified Scientist or Designated Supervisor, not\ntoday\u2019s date.&nbsp; The Date of Approval must\nbe before the tentative start date or actual start date listed by the student\non Form 1A. &nbsp;<\/p>\n\n\n\n<p>These forms will remain in Pending Completion status until\nall required signatures are provided.<\/p>\n\n\n\n<p><strong>How to check your signature<\/strong><\/p>\n\n\n\n<p>Once you have completed and signed the form, click on \u201cReturn to forms manager\u201d in red font or on &#8220;Registration \/ Forms&#8221; and then on \u201cManage Supervisor Forms\u201d on the horizontal menu bar just below the black and white speckled area.
